The enrolment form is completed by the physician and sent to Takeda Patient Support. Patients can choose their preferred method of contact, and you can specify any other needs on the enrolment form.
How it works
- Complete the Takeda Patient Support enrolment form with your patient.
- Send the completed form to Takeda Patient Support (fax: 1-844-951-7284).
- A Care Manager contacts your patient within one business day.
- Your office is kept updated on each step of your patient’s treatment journey.
